Pacemaker mediated tachycardia with tachycardia related card

Abstract

Various adverse arrhythmias may be encountered in normally functioning pacemakers, including cross talk, pacemaker-mediated or endless loop tachycardia, AV desynchronization arrhythmia, etc. In some cases, the resulting arrhythmia may be deleterious to the patient. We report a patient who presented with pacemaker mediated tachycardia (PMT) accompanied with tachycardia related cardiomyopathy. The patient had a relatively long VA conduction time. PMT resulted from retrograde conduction from the ventricle to the atrium. The retrograde P wave was sensed and led to subsequent triggering of ventricular pacing, which resulted in the development of a repetitive sequence of ventriculo-atrial conduction, a so-called endless loop. This tachycardia was controlled by reprogramming the atrial refractory period. After that, the tachycardia related cardiomyopathy also improved.
